As participants in an ongoing research program, users of the Project Live Happy application are required to be informed of their rights as research participants and are entitled to a clear description of the research they are participating in.  On this page is a copy of the consent form that new users are required to complete within the smartphone application.  You may consent to participate in this research only if you are over the legal age limit of 18 years.  This project is overseen by the California State University, Bakersfield (CSUB), Human Subjects Institutional Review Board (HSIRB) under approved protocol 15-63.

Informed Consent

By completing this form, you are consenting to participate in an online study conducted by the SPARK lab at California State University, Bakersfield. The purpose of this study is to examine how certain beliefs and motivations impact a person’s ability to change their own level of happiness. After providing consent using this form, you will be asked to complete a battery of surveys and provide demographic data. Because there is a potential for this task to be somewhat draining, you will be allowed 24 hours to rest after completing this survey battery before you begin the program.

□ I consent to voluntarily participate in the research, knowing that I am about to complete several surveys and that I will be asked to complete more surveys as the study progresses.

Confidentiality: The data obtained will be held in the strictest confidence. Your name will never be asked and your email address will be encrypted and stored separately from all other data.

Voluntary Participation:  Participation in this study is completely voluntary and you may withdraw consent to participate at any time without any penalty. You may also skip any question you are not comfortable answering or ask that the data you provide not be included in the study.

Requirements of Participation: For complete participation, we ask that you complete the registration process, take a break of at least 24 hours, and then choose one of the three activities that will be offered. You will do that activity once a week for four weekly sessions. You may also switch activities at any point in time, without suffering penalty to your progress.  Each session should last less than half an hour. Additional surveys will be administered throughout this period and periodically one week, one month, and three months after you complete your last activity.

□ I have read the above statement and I understand what is expected of me, including that I must choose and engage in at least one of the provided exercises in order to receive CSUB psychology research participation credit, that I will be completing activities over four weeks, and I will occasionally be asked survey questions over the following three months.

Benefits to Myself and the Community: Previous research indicates that some of the exercises you are exposed to may, in fact, make you lastingly happier in some way. By consenting to participate in this study, you are also helping advance psychological research. If you are taking a psychology course offered at California State University Bakersfield you may be eligible to also receive credit towards that course (given permission of the instructor). Periodically, you will be offered opportunities to have your email entered into a drawing for cash rewards; the more frequently you participate, the more opportunities you will have to win. There are no perceivable health risks to participation in this study. You may complete as many activities that you would like, in whatever order you would like, whenever you so choose. Follow-up surveys will be administered periodically one week, one month, and three months after the completion of the activities.
